Enrollment Process and Admission Policies

EPIC Learning Center admits children from the ages of six weeks through 13 years of age without regard to race, culture, sex, religion, national origin, ancestry or disability. 

• Admission Meeting

  • Prior to a child’s admission, a scheduled meeting is required with the parent(s)/ legal guardian(s) to review admission requirements, acquaint the family to the academy, staff, daily schedule and procedures as well as discuss specific needs of the child.

  • The child will have the opportunity to participate in the classroom activity during this meeting.

• Insurance Information

• Driver’s License

• Child’s Immunization Records

• Emergency contact address and phone numbers

• Registration Fee

• Materials Fee

• One Week Deposit

 

Your child’s file- Required forms and information

• Enrollment Application

• Emergency Contact Form

• Health Assessment

• Financial Agreement

• Civil Rights Compliance Form

• A copy of IEP or IFSP (if applicable)
