Enrollment & Admission

Who We Serve:
EPIC Learning Center welcomes children 6 weeks to 13 years in an inclusive environment, without regard to race, culture, sex, religion, national origin, ancestry, or disability.

Step 1: Schedule an Admission Meeting

Meet with our staff to:

• Review enrollment requirements

• Learn about our daily schedule and procedures

• Discuss your child’s needs

• Let your child participate in classroom activities

Step 2: Bring These Items

• Insurance info & driver’s license

• Child’s immunization records

• Emergency contacts

• Registration fee, materials fee, one-week deposit

Step 3: Complete Required Forms

• Enrollment application

• Emergency contact form

• Health assessment

• Financial agreement

• Civil Rights Compliance form

• IEP/IFSP if applicable
